MANAGER INFORMATION

Requirements for all team managers are listed below.

Please complete all PA Act 153 clearances BEFORE NCSI. You will need to upload your completed PA Act 153 clearances to NCSI.
All PA Act 153 clearances are valid for 5 years.

A Volunteer Affidavit can only be used in lieu of an FBI Check by anyone who has lived in PA for the last 10 consecutive years AND is participating as a volunteer with USA Hockey

NCSI Screening

YOU WILL NEED

Results of PA Child Abuse clearance
Results of PA Criminal History clearance
PA Fingerprint verification or notarized affidavit
Your USA Hockey number
Driver's license
There is a $30 fee to submit your NCSI clearance.

SafeSport Training for Managers

 

All team managers MUST complete SafeSport training.

 

SafeSport training must be completed every season.
